Office visits are by appointment only. If you can't keep an appointment, please give us 24 hours notice. Despite schedules, emergencies may cause delays. If your appointment is affected we do apologize, we know that your time is valuable.

To keep your medical records updated, bring a list of all current medications and dosages (or the bottles) with you.

If an emergency arises during office hours, you may call the office at 937-223-4461. After hours and on weekends call the Medical Society at 937-463-1729. If at any time a life threatening situation should occur, call 911.

Please bring your insurance cards to your appointment. Call your family doctor if a referral is needed.
